TNY267 problem.

Hello,
I'm producing universal 5V 1,5Amp power supplies. I'm having problems using TNY267. About 2% of boards I have TNY exploded. Mains AC Voltage is about 235V.
If you see picture imagem.jpg, you will notice that input rectifier signal tracks are damaged.
I would like you to analize what am I doing wrong with this design.
