LNK623PG--Why the voltage output much lower than expected?

We design the power circuit with LNK623PG.
There are two output designed:XVA(12V)and V3.3, but we just get 4V for XVA and 1V for V3.3.

Both two output voltages are just one third of expected.
Is the duty cycle set wrong? But how to change?

Please design or verify your design with our PIXLs Designer. You have to pay particular attention to your feedback winding location, voltage and resistors.

We follow AN-37, but still the out put is not stable. 3.3V @ 0.5A, we only applied 50mA load and Output Voltage is jumping from 1.5V to 8V
any recomendaiton to sort this issue.
