Output Ringing Problem in InnoSwitch3-CE dual output Design

Hi Team,
We have designed as per power integration simulation tools.
Our design has two outputs 12V 2A and 5V 3A.
I have attached the simulation report and we have made PCB and circuit as per that.
But we are not getting proper output. I have attached the waveform for the same.

I am not seeing any attachment. Can you also provide the schematic diagram and layout if possible?

We have tune snubber,alos output capacitor, and LC network on output side but we can not resolve it.
