problem with AC-DC conversion using LNK6666E

hi, 

   we design SMPS using following specifications.

Application:  Industrial Instrumentation,automation,pharma machinery

Input Type: Universal

Output Voltage: 15 VDC 

Output Current: 2A

Output Power: 30 W

part used: lnk6666e

also

following are some list of problem,

1. not getting output

2. out put fluctuates.

3. when smps  not powered up, using tester when we touch to pin no. 1 PD smps works.

4. output voltage drops.

and some more but not in all smps, we face problem in nearly 30 to 40%.

kindly go through attached schematic,and transformer data sheet pcb layout.

and suggest the corrective action. we are not able find out exact issue problem where it is at design or pcb layout or at component value / selection.
