TNY264

We made a 3 phase (3x230v RMS) power supply using TNY264 and FQD2N90 mosfet (to overcome high voltage on the switch).
You can find the schematic and normal condition signals in the attachment in the attachment.
We produced 6 PCBs and they were under test (1x230 v RMS input and 100mA load) during the past 18 months. Now I found out that two of them failed working properly.
The first one was not working at all. When we replaced its TNY264 with a new one, it started working normally. All the temperatures and signals are ok.
The second one was working normally for a while but its temperature was rising and as soon as it exceeded 85 degree centigrade it stop working (probably internal thermal shutdown) replacing its TNY264 with a new one, it started working normally. All the temperatures and signals are ok.
Please let me know your idea about the failure and its reason.

When you mention those 2 parts were not working, did you check if the part switch or not(auto-restart we call in datasheet). In other words, can you see any rise in your output when you have a light load.

One of our power was off so we don't have any output.
Another power is on for a few minutes then turns off (switch for short time and then off)You also mention the temperature effects on the perfromance. What's the temperature of your device for normal operation? Is it below 85C? Again for the second fail part, is it latch off(no swicthing) or auto-restart(switch for short time and then off)?

For your design, can you check what's the maximum voltage across the primary MOSFET and the maximum current through the drain during normal operation and startup with input at maximum voltage.
For your application, is it possible to have the surge condition and what is the maximum voltage across the MOSFET during surge condition.
