Looking for 90-320Vac input, 12Vdc output 3 to 5W reference design

Can you provide some reference design which is 90-320Vac input, 12Vdc output (3 to 5W) with small foot prints?

Efficiency appers to be only 50%. Do you have any other design with higher efficiency?
